Trelegy Ellipta is a prescription medication that is classified as a long-acting beta agonist (LABA).
What is it used for?
Trelegy Ellipta contains 3 different types of medications formulated in the same inhaler:
- Inhaled Corticosteroid (ICS) (Fluticasone)
- Long acting Beta- agonist (LABA) (Vilanterol)
- Long acting muscarinic antagonist (Umeclidinium)
These medications function to improve the symptoms associated with Chronic Obstructive Pulmonary Disease (COPD) and as a maintenance therapy for patients with asthma.
This medication should not be used for acute relief of bronchospams or asthma attacks.
How does it work?
The fluticasone component acts an anti-inflammatory agent, the umeclidinium inhibits the M3 receptors which causes bronchodilation, and the vilanterol increases the cyclic adenosine monophosphate levels, which causes the bronchial smooth muscles to relax.
How do I take Trelegy Ellipta?
Consult the provided package instructions for complete directions, listed is a set of abridged directions:
- The inhaler device provided has a grey cover, each time this cover is rotated to the side of the mouthpiece there will be a distinct click. This action primes a dose. Ensure you do not return the cover back to the original position without using your dose as this will waste a dose. It is not possible to accidently take a double dose.
- Once you have exposed the mouthpiece appropriately, exhale fully away from the inhaler, place your lips around the mouthpiece and inhale slowly and deeply through your mouth.
- Ensure you do not cover the air vents with your fingers.
- Remove the inhaler from your mouth and hold your breath for as long as is comfortable (3 to 4 seconds)
- Exhale slowly and gently away from the inhaler, return the cover back to its original position and store appropriately.
- The counter should be reduced by 1 after the dose.
- Rinse your mouth with water, do not swallow the water.

There is increased risk of pneumonia in patients with COPD using this medication, if you develop signs and symptoms of pneumonia contact your doctor.
If you experience a paradoxical bronchospasm while taking Trelegy discontinue and switch to an alternative therapy.
This medication may put you at risk for worsening of infections if you have existing tuberculosis, fungal, bacterial, viral, parasitic infections, or ocular herpes simplex ensure your prescriber is monitoring the progression closely.
If you are transitioning from an oral corticosteroid to this medication ensure you understand the transition instructions as inappropriate execution can put you at risk for complications.
Patients taking beta-blockers should notify their doctor if they have increased symptoms of increased heart rate, blood pressure, or chest pain.
Patients at risk for or have existing decreased bone mineral density should have their bone mineral density evaluated regularly.
If you have existing glaucoma or cataracts, notify if you experience worsening symptoms while on this medication.
If you have existing problems with urinary retention or prostatic hyperplasia notify your doctor if you experience symptom worsening.
What are the side effects of Trelegy Ellipta (fluticasone/umeclidinium/vilanterol inhalation powder)?
Some common (≥1%) side effects include:
- Upper respiratory Tract infection
- Oral Candidiasis
- Back Pain
- Arthralgia (Joint pain)
- Urinary tract infection
- Oropharyngeal pain

Does this medication interact with other drugs, foods, or alcohol?
Fluticasone is metabolized by CYP3A4, as a result, medications that are strong inhibitors of this enzyme should be used with caution.
MAOI’s and TCA should be used with extreme caution as they may potentiate the vascular effect of vilanterol.
Patients on beta-blockers should be cautious about this medication as it may potentiate a bronchospasm or decrease the effectiveness of the beta blocker.
How should this medication be stored?
Trelegy should be stored at room temperature between 68°F and 77°F
- If you experience acute symptoms of COPD use your provided emergency inhaler to improve symptoms, DO NOT use Trelegy for these symptoms.
- Monitor the frequency that you require doses from your emergency inhaler when using Trelegy as this will help the provider evaluate the effectiveness of your therapy.
- Make sure you rinse your mouth after each dose with water as this reduces your risk of developing an infection in your mouth and throat.
Can I just stop taking Trelegy?
Do not discontinue Trelegy unless otherwise instructed by your provider as this puts you at an increased risk for significant worsening symptoms.
Can you take Trelegy more than once a day?
Trelegy should be dosed as instructed by your provider. Trelegy was not studied as a more than once daily medication it should not be used more than once daily as this could put you at risk for side effects.
How quickly does Trelegy work?
The effectiveness of Trelegy is generally monitored through FEV1. Some patients show significant improvement after the first dose, others may take a few weeks to notice significant effects.
Can people with asthma take Trelegy?
Yes, Trelegy carries an FDA indication as a maintenance therapy for patients with asthma. This medication is not indicated for the treatment of acute asthma attacks, use your provided emergency inhaler to treat these symptoms.
