Toprol XL (Metoprolol Succinate) is a beta blocking agent used for the treatment of hypertension, angina pectoris (chest pain), and heart failure. 

How does it work?

Through competitive antagonism of catecholamines at peripheral and cardiac adrenergic receptors, it has antihypertensive effects because there is reduced sympathetic outflow, and suppression of renin activity.

Patients with respiratory conditions, such as asthma, should exercise caution with this medication as it may affect how effective medications like albuterol inhalers work. 

Patients undergoing non-cardiac surgery should wait to initiate Toprol XL (Metoprolol Succinate) until post-surgery as it can cause significant bradycardia and hypotension. If you are already maintained on Toprol XL (Metoprolol Succinate) do not discontinue before surgery unless otherwise directed. 

Patients with diabetes should exercise caution while on Toprol XL (Metoprolol Succinate) as it can mask the presentation of hypoglycemia by regulating high heart rates. 

Patients with a history of thyrotoxicosis should avoid abrupt discontinuation as it may precipitate a thyroid storm. 

Patients with a history of anaphylactic reactions should be extra vigilant while taking Toprol XL (Metoprolol Succinate) as it has been found to reduce the effectiveness of epinephrine utilized as rescue therapy. 

If you are already on a calcium channel blocker (verapamil/diltiazem) you are at an increased risk of developing significant bradycardia.

Toprol XL (Metoprolol Succinate) is metabolized by CYP2D6, medications that are inhibitors of this enzyme are likely to increase the effect of metoprolol.
