What is Saphris (Asenapine)?

Saphris (Asenapine) is a prescription medication that is classified as an atypical antipsychotic.

Uses

What is Saphris (Asenapine) used for? 

It is used to treat schizophrenia in adults and bipolar 1 disorder in patients aged 10 and up. For the treatment of Bipolar 1 Disorder, it can be used as acute monotherapy for mixed or manic episodes, in combination with lithium or valproate in adults, and as maintenance monotherapy in adult patients.

How does it work? 

Saphris helps treat the symptoms of bipolar 1 disorder by antagonizing D2 and 5-HT2a activity.

How do I take Saphris (Asenapine)?

This medication is taken sublingually which means that you should allow the tablet to dissolve under the tongue. This medication should dissolve fairly quickly. Make sure your hands are dry when handling this medication as moisture may cause the tablet to dissolve prematurely. Do not eat or drink anything for at least 10 minutes after taking a dose of this medication.

What dosages are available?

Saphris (Asenapine) is available as brand and generic 2.5 mg, 5 mg, and 10 mg sublingual tablets.

What is the recommended dosage?

Typically one 5 mg dose is given twice daily but take as directed by your doctor.

What is the maximum dosage?

The maximum recommended dosage is 20 mg/day.

What if I miss a dose?

If you miss a dose of this medication, take your dose as soon as you remember unless it is time for your next scheduled dose. In that case, just take your scheduled dose and skip your missed dose but do not double your dose to compensate.

Warnings & Precautions

What should you know before you take Saphris (Asenapine)?

In elderly patients with dementia-related psychosis, there is an increased risk for cerebrovascular events

If symptoms of neuroleptic malignant syndrome emerge, immediately discontinue

There is a risk for tardive dyskinesia; discontinue if appropriate

This medication may cause metabolic changes. Monitor for diabetes, dysplipidemia, and/or weight gain

There is a risk for orthostatic hypotension; monitor heart rate and blood pressure. Increased risk for orthostatic hypotension if dehydrated

In patients with low WBC, CBC monitoring may be warranted. If there is a significant, inexplicable decline in WBC count, consider discontinuing Saphris

This medication may cause QT prolongation; use caution in patients at high risk for QT prolongation and/or if on other QT-prolonging medications

Use caution in patients with seizures or are taking medications that lower the seizure threshold

Use caution when driving or operating heavy machinery

Use caution if on other anti-hypertensive medications as Saphris can cause hypotension

If used concomitantly with paroxetine, reduce paroxetine dose by 50%

How should this medication be stored?

Keep Saphris (Asenapine) in the original container and store it in a dry place at room temperature 15-30°C (59-86°F).

  • Discontinue this medication and seek medical attention if you experience any allergic symptoms after taking Saphris. Symptoms include: difficulty breathing, itching, facial swelling, or swelling of lips/tongue/throat.
  • Symptoms of neuroleptic malignant syndrome include: hyperpyrexia, muscle rigidity, altered mental status, and autonomic instability. Seek medical attention if any of these symptoms emerge.
  • Take this medication at the same time(s) each day to prevent missed doses. If you miss multiple doses consecutively, alert your doctor as they may want to re-taper the medication to prevent side effects.
