What is Janumet XR?

Janumet XR is a prescription medication that is classified as a combination product containing a dipeptidyl peptidase 4 (DPP-4) inhibitor (sitagliptin) and a biguanide (metformin).

What is Janumet XR used for? 

It is used for the treatment of type 2 diabetes mellitus.

How does it work?

The sitagliptin component lowers glucose levels in a glucose-dependent manner by inhibiting DPP4 which allows for increased levels of glucagon-like peptide-1 (GLP-1). Metformin increases glucose uptake and utilization to lower blood glucose levels.

How do I take Janumet XR?

Take your tablet whole by mouth with meals at the same time every day as directed by your doctor. Do not chew or crush the tablet. 

What dosages are available?

Janumet XR comes as a brand only in 50mg/500mg, 50mg/1000mg, and 100mg/1000mg ER tablets (sitagliptin IR/metformin ER).

What is the recommended dosage?

The dose differs based on patient-specific factors, for dosage recommendation refer to your doctor.

What is the maximum dosage?

The maximum recommended dosage is 100mg sitagliptin/2000mg metformin per day.

What if I miss a dose?

Take the medication as soon as possible if you have missed a dose. If it is almost time for your next dose, skip the missed dose and return back to your regular schedule. Never double the dose.

What should you know before you take Janumet XR?

Severe/disabling arthralgia has been reported with DPP-4 inhibitors. Onset can occur within a day to years after starting therapy. 

Bullous pemphigoid development has been associated with DPP-4 inhibitors.

Rare hypersensitivity reactions can occur, including Steven-Johnson syndrome.

Acute pancreatitis cases have been reported with use. 

Caution in renal function. Dose adjustment/discontinuation of therapy may be needed.

• eGFR > 45 mL/minute/1.73m2: No dose adjustment needed. 
• eGFR 30 to 45 mL/minute/1.73m2:
  • Initiation: Not recommended.
  • Continuation: A metformin dose reduction of 50% is needed (maximum 1 g/day) and limit the dose of sitagliptin to 50 mg once daily. 
• eGFR < 30 mL/minute/1.73m2: Use is contraindicated. 

If acute kidney injury occurs during therapy, the patient should hold the medication due to the metformin component.

Worsening renal function has been reported with sitagliptin.

Vitamin B12 deficiency has been associated with long-term metformin use. Vitamin B12 serum concentrations should be periodically monitored with long-term therapy.

U.S. Boxed Warning: Metformin-induced lactic acidosis have been reported in postmarketing cases that have resulted in death. The risk is increased with renal dysfunction.

What are the side effects of Janumet XR?

Some common side effects include:

• Abdominal pain, diarrhea, nausea
• Headache
• Upper respiratory tract infection

Does this medication interact with other drugs, foods, or alcohol?

Avoid use with alcohol due to increased risk of lactic acidosis with metformin (Risk Category X).

This medication should be stored between 20-25°C (68-77°F). Excursions are allowed to be stored between 15-30°C (59-86°F)
