- Initiation: Not recommended.
- Continuation: A metformin dose reduction of 50% is needed (maximum 1 g/day) and limit the dose of sitagliptin to 50 mg once daily.
- eGFR < 30 mL/minute/1.73m2: Use is contraindicated.
If acute kidney injury occurs during therapy, the patient should hold the medication due to the metformin component.
