What is Glyxambi (Empagliflozin/Linagliptin)? 

Glyxambi (Empagliflozin/Linagliptin) is a prescription medication that is classified as a combination of a sodium-glucose cotransporter 2 (SGLT2) inhibitor (empagliflozin) and a dipeptidyl peptidase 4 (DPP-4) inhibitor (linagliptin).

Uses

What is Glyxambi used for? 

It is used for the treatment of type 2 diabetes mellitus. To note, empagliflozin is indicated for risk reduction of cardiovascular mortality in adults with established cardiovascular disease and type 2 diabetes mellitus.

How does it work?

Empagliflozin increases urinary glucose excretion while linagliptin increases the levels of incretin hormones which stimulates glucose-dependent insulin release.

How do I take Glyxambi?

Take Glyxambi by mouth with or without food at the same time every morning as directed by your doctor.

What if I miss a dose?

Warnings & Precautions

What should you know before you take Glyxambi (Empagliflozin/Linagliptin)?

The Institute for Safe Medication Practices (ISMP) includes Glyxambi on its list for drugs that are at increased risk of causing significant patient harm when used in error.

Severe/disabling arthralgia has been reported with DPP-4 inhibitors. Onset can occur within a day to years after starting therapy.

There have been reports of increased bone fractures with SGLT2 inhibitors in some clinical trials, but meta-analysis data of empagliflozin has not shown an increased risk of bone fractures.

Hypersensitivity reactions can occur, usually within hours to days of starting therapy. Stop therapy and treat appropriately if hypersensitivity reactions occur.

Empagliflozin can cause hypotension due to intravascular volume depletion (increased risk in those with renal dysfunction).

Necrotizing fasciitis (Fournier gangrene), although rare and serious, has been reported in patients taking empagliflozin. Patients with malaise along the genitil, fever, or tenderness, erythema, perennial pain, or swelling should be assessed for necrotizing fasciitis. Stop the medication immediately if the patient develops this.

There have been reports of pancreatitis in those taking linagliptin. Glyxambi has not been studied in those with a history of pancreatitis.

Use of Glyxambi may increase the risk of genital mycotic infections. Uncircumcised males and patients with a history of these infections are at increased risk.

Kidney dysfunction may require a dose adjustment

  • eGFR > 45 mL/minute/1.73m2: No dose adjustment needed.
  • eGFR 30 to 45 mL/minute/1.73m2: Do not initiate therapy. If patients are already on it, discontinue the medication if eGFR is persistently < 45 mL/min/1.73m2. Also refer to the individual agents in Glyxambi (empagliflozin and linagliptin).
  • eGFR < 30 mL/minute/1.73m2: Contraindicated
  • End-stage renal disease (ESRD) or dialysis: Contraindicated

Treatment with a DDP-4 inhibitor has been associated with heart failure. However, a large-randomized, double-blinded study in patients with established cardiovascular disease and type 2 diabetes showed that the occurrence of the primary composite outcome (CAD, atherosclerotic peripheral arterial disease, or ischemic cerebrovascular disease) and the occurrence of the primary composite cardiovascular outcome (nonfatal MI, nonfatal stroke, cardiovascular death, or hospitalization for unstable angina) with sitagliptin was found to be noninferior to placebo. The rate of hospitalization for heart failure did not differ between the two groups. The American Diabetes Association (ADA) suggests that DPP-4 inhibitors (except saxagliptin) can be considered in patients with heart failure.

Interactions

Does this medication interact with other drugs, foods, or alcohol?

Use with sulfonylureas may increase the risk of hypoglycemic effect of the sulfonylurea. A dose decrease of the sulfonylurea should be considered when starting a DPP-4 inhibitor (Risk Category D).

How should this medication be stored?

Store your Glyxambi at 25°C (77°F). Excursions are allowed to be stored between 15°-30°C (59°-86°F).
