Entresto (Sacubitril/Valsartan) is used to reduce the risk of cardiovascular death in patients with chronic heart failure and to help treat symptoms of heart failure.

How does it work?

It inhibits neprilysin which increases levels of natriuretic peptides and inhibits angiotensin-II dependent aldosterone release. 

How do I take Entresto (Sacubitril/Valsartan)?

You should take Entresto (Sacubitril/Valsartan) as instructed by your physician. It is important to note, your dose may need to be adjusted several times to find what works best for you.

What dosages are available?

Entresto (Sacubitril/Valsartan) is available as brand name 24mg/26mg, 49mg/51mg, and 97mg/103mg tablets

What is the recommended dosage?

Generally, it is dosed 1 tablet by mouth twice daily but take as directed by your doctor. 

What is the maximum dosage?

The maximum recommended dosage is 194/206mg/day.

What if I miss a dose?

If you miss a dose, take it as soon as you remember. If it is relatively close to when your next dose is due, wait until then and take a normal dose. Do not take an extra dose to make up for the missed dose. 

What should you know before you take Entresto (Sacubitril/Valsartan)?

Patients should monitor for signs and symptoms of angioedema (swelling of the tongue) as this is a potentially serious side effect of Entresto (Sacubitril/Valsartan).

Hypotension is a common side effect of Entresto (Sacubitril/Valsartan), talk with your doctor if you have significant dizziness with Entresto (Sacubitril/Valsartan). 

Patients with existing renal dysfunction or those at risk of developing it should be monitored regularly for renal injury.  

What are the side effects of Entresto (Sacubitril/Valsartan)?

Some common (≥5%) side effects include:

• Hypotension
• Hyperkalemia 
• Cough

• Renal Failure

Does this medication interact with other drugs, foods, or alcohol?

Patients with an eGFR <60 should avoid Entresto (Sacubitril/Valsartan) in combination with aliskiren. 

Potassium-sparing diuretics should be used with caution as they can increase a patient’s risk of developing hyperkalemia. 

Concomitant use of other agents known to cause kidney damage should be limited while on this medication. 

Patients taking lithium should be closely monitored while starting or changing dosages. 

How should this medication be stored?

Entresto (Sacubitril/Valsartan) should be stored at room temperature (68°F to 77°F). Protect Entresto (Sacubitril/Valsartan) from moisture. 

• Patients transitioning from an ACEi therapy to Entresto (Sacubitril/Valsartan) should ensure that they have a 36 hour washout period before starting Entresto (Sacubitril/Valsartan). 
• In order for this medication to reach maximum effectiveness, it will need to be titrated to a maximum tolerated dosage. 
• Talk with your doctor if you become or are trying to become pregnant.
