What is Dulera?

Dulera is a prescription medication that is classified as a combination corticosteroid (mometasone) and long-acting beta2-adrenergic agonist (formoterol) inhaler.

Uses

What is Dulera used for? 

It is used for the treatment of asthma in those 5 years of age and up.

How does it work?

The mometasone exhibits anti-inflammatory action and the formoterol increases cyclic adenosine monophosphate levels which causes bronchial smooth muscle relaxation.

Directions

How to administer Dulera?

  • Step 1: You must prime your inhaler if it is the first time you are using it.
    • To prime your inhaler, you will hold it upright and spray 4 actuations/puffs into the air and away from your face.
    • You will need to shake the inhaler well before priming.
    • If it has been 5 or more days since you have used your inhaler, you will need to prime it again (follow the same steps above).
    • The dose counter will show ‘64’ or ‘124’ initially. After you prime your inhaler, the dose counter will read either ‘60’ or ‘120’ as you have sprayed 4 puffs to prime.
  • Step 2: Shake your Dulera inhaler before each administration.
    • Remove the cap from the mouthpiece and check for any objects.
    • Ensure that the canister is loaded into the canister fully.
    • Shake your Dulera inhaler well before each use.
  • Step 3: Inhalation.
    • Breathe (through your mouth) as much as you comfortably can, pushing out as much air from your lungs as possible. Hold your inhaler upright and put the mouthpiece into your mouth. Then, close your lips around the mouthpiece.
    • Once you are done breathing in, hold your breath for as long as you comfortably can, up to 10 seconds. Then, remove the mouthpiece from your mouth while at the same time, breathing out through your nose with your lips closed.
  • Step 4: Repeat.
    • Wait at least 30 seconds before your second puff of your Dulera following the same steps prior.
    • Place the cap over the mouthpiece after you have finished your administration right away.
    • After your 2 puffs of Dulera, be sure to rinse your mouth with water and spit out. Do not swallow the water.

Once the counter reads ‘20’, that means it is time to get a refill of your Dulera or get a new prescription. If the counter reads ‘0’, that means it is time to throw away your inhaler. It may not feel empty and may continue to spray puffs, but that will give you the incorrect dose of the medication if you use it.

What dosages are available?

Dulera is only available as brand and comes in mometasone 50mcg and formoterol 5mcg per inhalation (13 g), mometasone 100mcg and formoterol 5mcg per inhalation (8.8 g, 13 g), and mometasone 200mcg and formoterol 5mcg per inhalation (8.8 g, 13 g).

There is no generic equivalent to Dulera.

What is the recommended dosage?

The recommended starting dosage is based on previous asthma therapies and disease severity. This medication is dosed 2 inhalations twice daily.

What is the maximum dosage?

The maximum daily dosage is 800mcg/20mcg

What if I miss a dose?

Skip your missed dose of Dulera and continue with your regular dosing schedule. Do not take more puffs than prescribed.

Warnings & Precautions

What should you know before you take Dulera?

Caution in patients with major risk factors for decreased bone density such as a family history of osteoporosis, tobacco use, advanced age, prolonged immobilization, or chronic use of drugs that can decrease bone mass (e.g., oral corticosteroids or anticonvulsants).

Monitor patients with hepatic dysfunction as it can cause an increase in mometasone levels.

Oral candidiasis (Candida) infections have been reported. Patients should be instructed to rinse their mouth out with water and spit it out to decrease the risk of this occurring.

Interactions

Does this medication interact with other drugs, foods, or alcohol?

Avoid use with Aldesleukin, another long-acting beta2-adrenergic agonist, nonselective beta-blockers, and desmopressin (Risk Category X).

How should this medication be stored?

Your Dulera inhaler should be stored at 20°-25°C (68°-77°F). Excursions are allowed at 15°-30°C (59°-86°F).
