What is Asacol? 

Asacol is a prescription medication that is classified as a 5-aminosalicylic acid derivative. 

Uses

What is Asacol used for? 

It is used for the treatment and maintenance of moderately active ulcerative colitis in adults.

How does it work?

The mechanism isn’t well defined but mesalamine’s potential for blocking cyclooxygenase and prostaglandin synthesis in the colon is theorized to improve inflammation.

How to take Asacol? 

Take your Asacol:

  • Formulated without dibutyl phthalate [DBP]: By mouth on an empty stomach at least 1 hour before or 2 hours after your meal. Do not break, cut, chew, or crush the tablet.

Asacol formulated with DBP has been discontinued in the U.S. for > 1 year, but is still available in other countries, such as Canada.

What dosages are available?

Asacol comes as a brand or generic delayed release 400mg and 800mg tablet.

What is the recommended dosage?

For moderately active ulcerative colitis the manufacturer recommends 1600 mg by mouth three times a day for 6 weeks.

What is the maximum dosage?

The maximum recommended dosage of the Mesalamine 400 mg tablets is 4.8g/day divided into 3 doses.

What if I miss a dose?

Take your dose of Asacol as soon as possible, but if it is almost time for your next dose, skip the missed dose and return to your normal dosing schedule. Never double doses.

Warnings & Precautions

What should you know before you take Asacol?

Use caution in patients with renal dysfunction, a history of renal disease, or those on other nephrotoxic medications.

Use caution in patients that are predisposed to any of these conditions as they have been reported: pericarditis and myocarditis (mesalamine-induced cardiac hypersensitivity reactions).

Use with caution in the elderly (> 65 years of age) as postmarketing reports and uncontrolled studies suggest an increased incidence of blood dyscrasias in those > 65 years of age.

Use with caution in patients with pre-existing skin conditions as severe photosensitivity reactions have been reported.

Evaluate the risk vs benefits of therapy in those with hepatic dysfunction as hepatic failure has been reported with Asacol use.

An intolerance syndrome can develop, with symptoms from cramping, bloody diarrhea, abdominal pain, which can be difficult to differentiate from an exacerbation. Monitor for any worsening of symptoms and stop the medication immediately if this syndrome occurs or is suspected.

Asacol contains ~4.9 mg of iron per 800 mg tablet.
