What is Anafranil? 

Anafranil is a prescription medication that is classified as a tricyclic tertiary amine, antidepressant.

What is Anafranil used for?

It is indicated for the treatment of obsessive-compulsive disorder (OCD).

How does it work?

Its exact mechanism in how OCD is managed is unclear but it inhibits the reuptake of serotonin which can contribute to the effects of this medication.

How do I take Anafranil?

The initial dose can be increased as tolerated over the first two weeks, in divided doses (~100 mg/day). Your doctor will be titrating your medication. Do not increase or decrease your dose yourself. 

Your maintenance dose of Anafranil will be determined by your doctor, after the initial titration. 

You may divide your Anafranil dose and take with food to help decrease the risk of any gastrointestinal (GI) side effects

What dosages are available?

Anafranil comes in a capsule, as either brand or generic (25 mg, 50 mg, 75 mg). 

What is the recommended dosage?

TheInitial dosing is typically 25 mg by mouth once daily but take as directed by your doctor. 

What is the maximum dosage?

The maximum recommended dosage is 250 mg/day.

What if I miss a dose? 

Take your next dose of Anafranil as soon as possible. Skip the missed dose and return to your regular dosing schedule if it is almost time for your next dose. Never double doses.

What should you know before you take Anafranil?

Anafranil falls under the Beers Criteria and is potentially an inappropriate medication. 

Caution should be used in patients with BPH, xerostomia, urinary retention, visual problems, decreased GI motility, or paralytic ileus as Anafranil may cause anticholinergic effects (e.g., blurred vision, urinary retention, xerostomia, constipation).

Vomiting is 2- to 3-fold more common in children compared to adolescents and is more common in adolescents compared to adults. To help decrease the risk of vomiting, Anafranil doses can be divided and taken with food initially. 

U.S. Boxed Warning:  Antidepressants can increase suicidal thinking/behavior in those 18 to 24 years of age with major depressive disorder (MDD) and other psychiatric disorders.

What are the side effects of Anafranil?

The most common side effects of Anafranil include:

• Dizziness, drowsiness, headache, insomnia, nervousness, myoclonus
• Diaphoresis
• Change in libido, weight gain 
• Xerostomia, constipation, nausea, dyspepsia, anorexia, diarrhea, abdominal pain, increased appetite 
• Ejaculation failure, impotence, difficulty in micturition 
• Tremor, myalgia
• Visual disturbance
• Pharyngitis

Does this medication interact with other drugs, foods, or alcohol?

Abametapir may increase the serum concentration of CYP1A2 substrates, which Anafranil is. Combination use should be avoided (Risk Category X).

Aclidinium may increase the anticholinergic effects of Anafranil and combination use should be avoided (Risk Category X).

Anafranil may increase the vasopressor effect of alpha- and beta- agonists, and combination use should be avoided if possible (Risk Category D). 

Anafranil may decrease the antihypertensive effect of alpha2-agonists and combination use should be avoided if possible (Risk Category D).

How should this medication be stored?

It should be stored at 20° to 25°C (68° to 77°F) and kept away from moisture.

• During a dose increase, you may divide your dose and take your Anafranil to decrease the risk of any GI side effects. Capsules can be opened to divide the dose, but do not chew the capsules.
• Never stop taking your antidepressant without first speaking to your doctor. 
• Do not take an MAO inhibitor within 2 weeks of stopping your medication unless directed to do so by your doctor.
• Call your doctor right away if you or your family is experiencing any of the following symptoms, especially if they are new, worsening, or are worrisome:
  • Thoughts about dying/suicide
  • Suicide attempts 
  • New or worse depression, anxiety, or irritability 
  • Feeling restless or agitated
  • Insomnia 
  • Panic attacks 
  • Acting violent, angry, or aggressive 
  • Mania 
  • Any other unusual changes in mood or behavior
