What is Amoxil (Amoxicillin)?

Amoxil is a prescription medication that is classified as a penicillin antimicrobial agent.

Uses

What is Amoxil (Amoxicillin) used for?

It is used to treat certain infections caused by specific bacteria. These infections include:

  • Ear, nose, and throat infections
    • IR: Infections due to beta-lactamase-negative Streptococcus spp. (alpha- and beta-hemolytic isolates only), Staphylococcus spp., Streptococcus pneumoniae, or Haemophilus influenzae.
    • ER: Treatment of tonsillitis and/or pharyngitis due to Streptococcus pyogenes in those 12 years of age and up 
  • Helicobacter pylori infection
    • IR
    • Treatment of H. pylori 
  • Lower respiratory tract infections
    • IR
    • Treatment of infections due to 
  • Acute bacterial rhinosinusitis 
    • IR
    • Treatment of infections due to beta-lactamase-negative Streptococcus spp. (alpha- and beta-hemolytic strains only), Staphylococcus spp., S. pneumoniae, or H. influenzae.
  • Skin and skin structure infections

How does it work?

It binds to penicillin-binding proteins in the cell wall to inhibit bacterial cell wall synthesis which results in cell death.

How do I take Amoxil (Amoxicillin)?

What dosages are available?

Generic Amoxil comes in capsules, suspensions, chewable tablets, and ER tablets. Brand and generic Amoxil 775 mg ER tablets have been discontinued in the U.S. for over a year. 

What is the recommended dosage?

The usual dosing range of Amoxil are:

  • IR: 500 mg to 1 g orally every 8 to 12 hours
  • ER: 775 mg orally once daily 

These doses may vary based on patient-specific factors and based on the indication for use so use as directed by your doctor. 

What is the maximum dosage?

The maximum recommended dosage is 1,750 mg/day.

What if I miss a dose?

If you forget to take Amoxicillin, take it as soon as you remember. If it is relatively close to when your next dose is due, wait until then and take a normal dose. Do not take an extra dose to make up for the missed dose. 

Warnings & Precautions

What should you know before you take Amoxil?

Prolonged use of Amoxil can lead to superinfections, including pseudomembranous colitis and C. difficile-associated diarrhea (CDAD). 

Infectious mononucleosis has been reported in a high percentage of patients during Amoxil therapy. Avoid use of Amoxil in these patients.

Some dosage forms may contain sodium benzoate/benzoic acid, which is a metabolite of benzyl alcohol. Large doses of benzyl alcohol (≥99 mg/kg/day) have been associated with . Avoid dosage forms that contain benzyl alcohol derivatives or use with caution in neonates. See the manufacturer’s labeling for more information.

The chewable formulation of Amoxil may contain phenylephrine. See the manufacturer’s labeling for more information.

Epstein-Barr virus infection (infectious mononucleosis), cytomegalovirus infection, or  acute lymphocytic leukemia  increases risk for amoxicillin-induced maculopapular rash in pediatric patients. If a rash occurs, it is important to carefully evaluate its appearance.

Renal dysfunction may require dose adjustments and caution should be used. Dose adjustment is recommended in patients with a GFR < 30 mL/minute. Avoid use of the 775 mg ER tablet and 875 mg IR tablet in patients with a GFR < 30 mL/min or those who require hemodialysis.

What are the side effects of Amoxil?

The most common side effects of Amoxil include:

  • Headache
  • Diarrhea
  • Nausea
  • Vomiting
  • Vulvovaginal infection

Interactions

Does this medication interact with other drugs, foods, or alcohol?

Antibiotics can decrease the efficacy of BCG (intravesical) and the cholera vaccine. Use together should be avoided (Risk Category X).

Antibiotics can decrease the efficacy of Sodium Picosulfate and the typhoid vaccine (Live strain only). A change in therapy should be considered (Risk Category D). 

  • An alternative product for bowel cleansing before a colonoscopy should be considered in those who have recently or are being treated with an antibiotic. 
  • Avoid use of the live attenuated typhoid vaccine (Ty21a) in those being treated with systemic antibiotics. Vaccination should be postponed until 3 days of stopping the antibiotics. Avoid starting antibiotics within the last 3 days of the last vaccine dose.

Storage

How should this medication be stored?

Store at or below 20°C (68°F)

  • 500-mg capsules; 250-mg unreconstituted powder 

Store at or below 25°C (77°F)

  • 200-mg and 400-mg unreconstituted powder; 500-mg and 875-mg tablets;  200-mg and 400-mg chewable tablets

Medication should be dispensed in a tight container.

  • A side effect of Amoxil is a swollen tongue, which can become hairy/black.

Does Amoxil make you tired?

Fatigue is not listed as a side effect, although Amoxil can cause insomnia, in turn, causing you to feel tired. The frequency of the side effect of insomnia is not defined.

Does Amoxil have penicillin in it?
