Strong CYP3A inhibitors: Do not use Ranexa (Ranolazine) if you are taking ketoconazole, itraconazole, clarithromycin, nefazodone, nelfinavir, ritonavir, indinavir, and saquinavir.

Moderate CYP3A inhibitors: If you take a moderate inhibitor of CYP3A your dose of Ranexa (Ranolazine) should be limited to 500 mg twice daily. Examples include: diltiazem, verapamil, erythromycin, fluconazole, and grapefruit juice or grapefruit containing products.

CYP3A Inducers: Do not use Ranexa (Ranolazine) if you are taking rifampin, rifabutin, rifapentin, phenobarbital, phenytoin, carbamazepine, and St. John’s wort.

Patients taking simvastatin concurrently should be limited to a dose of 20mg once daily.
